The Transitional Care Network (TCN) is a network that was created for Fairview Ridges Hospital, Fairview Southdale Hospital and the University of Minnesota Medical Center, Fairview, to create Fairview affiliated locations for short term rehabilitation care for Fairview patients.

The TCN clinical model provides on-site primary care to patients while they transition from the hospital to home or an alternative care setting.
