Fairview Partners has re-invented the house call for seniors who live in participating long term care facilities by providing a primary care team, consisting of a Fairview affiliated physician and gerontological nurse practitioner (GNP), who care for you right at your bedside.

The Fairview Partners long term care facility program was first established in 1995 through a grant by the Robert  Wood Johnson Foundation for innovative health system reform.

Seniors who would like to be a part of Fairview Partners have to enroll into the program. To help make the enrollment process as easy as possible, Fairview Partners has Enrollment Specialists who help with enrollment and work directly with you, your family and the long term care facility staff.
