A 68 bed facility, Villages of North Branch offers services for long term and short term care residents. Villages of North Branch is an Ecumen community.

Education and Training
Christa Hoiland has a medical degree from Yeshiva University Albert Einstein College of Medicine. She completed her internship and residency at the University of Minnesota Medical Center, Fairview.
Experience
Hoiland has been practicing with Fairview Chisago lakes Clinic for several years. She also provides care to Fairview Partners patients at Villages of North Branch and Birchwood Health Care Center. She is also the medical director at both of these facilities.
Licensure and Certifications
Hoiland is board certified in both family and geriatric medicine. She is also a certified medical director.

Hoiland is a member of the American Academy of Family Physicians, Minnesota Academy of Family Physicians, American Geriatrics Society, Minnesota Medical Association, American Medical Association, American Medical Director’s Association, Minnesota Medical Director’s Association and the Christian Medical Dental Association.
Interests
In her free time, Hoiland enjoys dance, cross-country skiing, hiking, biking, roller-blading, cooking and spending time with her family.

TOM LEAF, M.D.
Fairview North Branch Clinic

“It has been my privilege to serve the North Branch community for more than 20 years. I enjoy the wide variety of medical problems I encounter each day, caring for all ages from newborns to the elderly. I am fortunate that I can continue to care for individuals who move to Green Acres Country
Care Center for rehabilitation or for a more permanent residence.”

Education and Training
Thomas Leaf has a medical degree from the University of Minnesota Medical School. He received his family practice residency training at the University of Minnesota Medical Center, Fairview.
Experience
Leaf has been a family practice physician at the Fairview Lakes North Branch Clinic for over 20 years. He also practices with Fairview Partners at Villages of North Branch where he has been providing care for more than 20 years. Leaf served as the chief of staff at Fairview Lakes Regional Medical Center from 2000 to 2004.
Licensure and Certifications
Leaf is board certified in family medicine.
Professional Associations
Leaf is a member of the Minnesota Medical Association, the American Medical Association, the Minnesota Academy of Family Practice and the American Academy of Family Practice.
Interests
Professionally, Leaf enjoys working with a broad spectrum of family practice including clinic, hospital obstetrics and nursing home care. He cares for patients of all ages, from infants to geriatrics. Personally, Leaf is married with four children and enjoys swimming, canoeing and bicycling.

Focusing on prevention, early detection and treatment of illnesses helps to maintain or improve the patient’s quality of life. I believe in working as a team with the patient, family, physician and nursing home staff, as each has a valuable perspective to offer.”

Education and Training
Sue Girard began her nursing career with a Bachelor of Science in Nursing from the University of Minnesota in 1978. Girard then went on to complete her Masters of Science in Nursing from the College of St. Catherine in 1998.
Experience
In 1999, after a year working in an acute rehabilitation setting at Bethesda Rehabilitation Hospital, Girard joined the Fairview Partners team and began providing care to long term care residents at several nursing homes in the metro area. In 2005, Girard began providing care at Fairview Partners member nursing homes up in the Fairview Lakes region. She currently provides care at Villages of North Branch and Golden Living of Rush City.
Licensure and Certifications
Girard is certified as a gerontological nurse practitioner by the American Nurses Credentialing Center (ANCC).
Personal Interests
Professionally, Girard has always been passionate about working with the geriatric population. She started working with this population as a public health nurse and enjoys the opportunity to continue to work with geriatric patients at Fairview Partners. In her free time, Girard enjoys cooking, outdoor activities, travel, camping and spending time with her husband and two sons.
