A 200 bed facility, Augustana Health Care Center of Apple Valley offers care services for long term and short term care residents. Augustana Health Care Center of Apple Valley is part of the Augustana Care Corporation.

JOHN ERICKSON, M.D.
Fairview Ridges Clinic

Education and Training
John Erickson received his medical degree from the University of Minnesota Medical School. He completed both his internship and residency at Valley Medical of Fresno, Fresno, CA.
Experience
Erickson has been a physician at Fairview Ridges Clinic since 1987. He has also been practicing with Fairview Partners at Augustana Health Care Center of Apple Valley for several years.
Licensure and Certifications
Erickson is board certified in internal medicine.
Professional Associations
Erickson is a member of the American College of Physicians and the American Society of Internal Medicine.
Interests
Erickson’s personal interests include outdoor activities with his wife and two children, golf, fishing and traveling.

Education and Training
Mary Homan received his medical degree from the University of Chicago Pritzker School of Medicine. She completed her residency at Ramsey County Medical Center, St. Paul, Minnesota.
Experience
Homan currently practices at Fairview Cedar Ridge Clinic and Fairview Farmington Clinic Additionally, Homan joined the Fairview Partners team in 2008 and provides care to Fairview Partners member residents at Augustana Health Care Center of Apple Valley.
Licensure and Certifications
Homan is board certified in family medicine.

KIRK SIMPSON , M.D.
Bloomington Lake Clinic

Education and Training
Kirk Simpson has a medical degree from the University of Minnesota Medical School.
Experience
Simpson has been a physician at the Bloomington Lake Clinic since 1975. He serves as a professor of clinical medicine at the University of Minnesota. He is also an educator for the family-practice residency program at Hennepin County Medical Center. Additionally, he has been providing care for Fairview Partners patients in both the long term and short term care settings as several metro nursing homes for many years.

Simpson is board certified in family medicine.
Professional Associations
Simpson is a member of the American, Minnesota and Hennepin County Academies of Family Practice.
Interests
Professionally, Simpson has particular interests in preventative medicine, public-health issues and cancer prevention and screenings.
In his free time, Simpson enjoys American Civil War history, sports, banjo-picking and spending time with his wife, Kathy. He has also been a volunteer for the American Cancer Society since 1981, serving as the Minnesota Division president from 1993-1995.

Education and Training
Maya Walkington-Stauber began her nursing career with a diploma from the Red Cross School of Nursing in Bern, Switzerland. She then received a Bachelor of Science in Nursing from Slippery Rock University in Pennsylvania and a Master of Arts in Nursing from the College of St. Catherine.
Experience
Walkington-Stauber has been practicing as a gerontological nurse practitioner (G.N.P.) for many years and has been with Fairview Partners since May 1996. She works in both the long term care and transitional care settings. She has also worked in clinical education for nursing staff and the elderly in the community and is a clinical instructor to G.N.P. students. Walkington-Stauber currently provides care at Augustana Health Care Center of Apple Valley and Ebenezer Ridges Care Center. She has been providing care at these two facilities since February 1998.
Licensure and Certifications
Walkington-Stauber is certified as a gerontological nurse practitioner by the American Nurses Credentialing Center (ANCC).
Professional Associations
Walkington-Stauber is a member of the American College of Nurse Practitioners and the American Medical Director Association (AMDA).
Personal Interests
In her free time, Walkington-Stauber enjoys gardening, reading, photography and spending time with her husband and cats.

LEEANN OLSON R.N., G.N.P.

Education and Training
LeeAnn Olson began her nursing career with an Associate of Science in Nursing from Anoka Ramsey Community College in 1992. Olson then went back to school and received her Bachelor of Science in Nursing from the College of St. Catherine in 2000, and her Master of Arts in Nursing from the College of St. Catherine in 2003.
Experience
Olson joined the Fairview Partners team in January 2007, but, she has been caring for the geriatric population since the young age of 14 when she became one of the first candy stripers at the nursing home in her home town. After her candy striper work, Olson was a nursing assistant for 13 years. She then went back to school and worked as a nurse in long term care and transitional care for 6 years and she then spent 3 years working with palliative medicine in an acute care setting at United Hospital as a GNP.
Olson currently provides long term care to Fairview Partners members at Augustana Health Care Center of Apple Valley and Providence Place.
Licensure and Certifications
Olson is certified as a gerontological nurse practitioner by the American Nurses Credentialing Center (ANCC).
Personal Interests
Professionally, Olson enjoys working with Dementia, Parkinson’s and other related disorders. Additionally, since her work with palliative medicine, she has become interested in pain management and the geriatric population.
Personally, Olson enjoys hiking the Superior National Forest trails, fishing and spending time with her family.
