To enroll in the long term care facility program, you must be a long term care resident at one of the Fairview Partners member facilities, be eligible for Medicare Parts A and B and be 65 years of age or older.

Residents with Medicare who privately pay for insurance need to already have or obtain UCare for Seniors insurance through UCare Minnesota.

Residents who are in the process of obtaining or already receive Medicaid (medical assistance) benefits would need to elect the Minnesota Senior Health Options (MSHO) benefit through Medica or UCare Minnesota.
