As a member of Fairview Partners you will receive care tailored for your particular health needs from a Fairview affiliated physician and gerontological nurse practitioner (GNP) team. They will be your team: one that knows you, your medical history, family history and health concerns.

The team will see you in the comfort of your room as often as is needed—no transportation required. In addition, if you need a specialist, Fairview Partners works to bring these specialists to your bedside as often as possible.

Fairview Partners is part of the Fairview continuum of care. The Fairview network provides access to specialists and hospital care. This model keeps communication between providers open to make sure you are receiving the best care possible.

In addition, with the Fairview Partners program, there is virtually no paperwork to fill out and no claim forms to submit.
