Fairview Partners has brought back the house call for seniors living in participating skilled nursing facilities by providing a Fairview doctor and geriatric nurse practitioner care team who visit you right at your bedside and take care of all your primary health care needs.

Fairview Partners is a not-for-profit organization that has been providing care to seniors since 1996 and was one of only 10 programs in the nation to receive a prestigious Robert Wood Johnson Foundation grant for innovative health system reform.

To be a part of Fairview Partners you need to enroll.  To make enrollment as easy as possible, Fairview Partners has Enrollment Specialists who work directly with you, your family and the skilled nursing facility staff to help you join.
