A:No, the Memory Loss Assessment Clinic does not replace a patient's primary care clinic; the memory loss assessments augment the patients' care.
Q: What insurance does the Memory Loss Assessment Clinic accept?
A:The Fairview Memory Loss Assessment Clinic is a participating provider for most local health plans and major insurance plans, as well as Medicare and Medicaid.
Q: As a family member will I have the opportunity to speak to the physician to voice my concerns about my loved one with memory loss?
A:Yes, family members/caregivers are a big help in filling in the gaps and getting a better understanding of the degree of memory loss. If your loved one agrees during the appointment, time will be made, otherwise, before the appointment you can speak to the nurse case manager about your concerns and she will relay this information to the physician.
Q: Will my primary care physician get a copy of what went on during the visit?
A:Yes, if you are in the Fairview system it will be through the electronic medical record, otherwise, it will be mailed or faxed to your primary care clinic.
Q: What can I expect out of the appointment at the Memory Loss Assessment Clinic?
A:The physician and nurse case manager will meet with you and your family/caregiver (if you bring them to the appointment) all together. Then the physician will meet with the family/caregiver if patient agrees and the nurse case manager will perform some screening tests with the patient. Once tests are completed everyone will meet together to discuss the next plan, whether that be more lab tests, imagining, medication or more cognitive testing and then follow up to discuss the diagnosis.
