To be part of the Fairview Partners community program, eligible individuals need to be at least 65 years of age or older and currently seeing a Fairview physician at a Fairview primary care clinic.

Additionally, eligible members need to part of the Minnesota Senior Health Options (MSHO) program, a benefit under Medicaid. This benefit would have to be elected under either Medica or UCare Minnesota.

There is no obligation. It you enroll in the Fairview Partners program and you are not completely satisfied, you may choose to disenroll at any time.
