Debra Lee, RN
Debra Lee, RN, received her degree in nursing from Inver Hills Community College and also has a degree in Occupational Therapy from Anoka Ramsey Community College. Lee has many years of experience working with geriatric patients in long term care facilities, Fairview Homecare and in case management. She joined the Fairview Partners team in 2006.

Joan Mattila, RN, PHN
Joan Mattila, RN, PHN, has a bachelor's degree in sociology from the University of Minnesota, and started her nursing career with a degree from Normandale Community College. She also received a degree in public health nursing from the College of St. Catherine. Mattila has been working with the geriatric community for many years, her experience includes home care, hospice care as well as working as a charge nurse at a community clinic. Additionally, Mattila was part of the Healthy Seniors Project. She joined the Fairview Partners team in 2007.

Catherine Nurmi, RN-BC, CMC
Catherine Nurmi, RN-BC, CMC, received her degree in nursing from the College of Saint Catherine and also has her certificate in gerontology. Nurmi has been a nurse for many years. Her experience includes nurse management and case management for both young adults and seniors in both the long term care facility and home care settings. She joined the Fairview Partners team in 2003.

Sharon Oliver, LSW
Sharon Oliver, LSW, received her bachelor's degree in social work from the University of North Dakota. Oliver has been working with the geriatric population for several years as both a long term care facility social worker and in case management. She joined the Fairview Partners team in 2004.

Lynn Radecki, RN, MA
Lynn Radecki, RN, MA, received her bachelor's degree in nursing from the University of Minnesota, and, received her master's degree in Health and Human Services Administration from the College of St. Mary. Radecki has been working in home care and case management in the Twin Cities area for more than ten years. She joined the Fairview Partners team in 2007.

Lynne Raymo, RN
Lynne Raymo, RN has worked in a variety of positions throughout her nursing career, beginning in the hospital and then pursuing her goal to work with the elderly. Prior to case management work with Fairview Partners, Raymo developed and opened assisted living programs, was a site supervisor for a Fairview clinic, and worked as a case manager for home care. She joined the Fairview Partners team in 2004.

Lisa Remer, LSW, MA
Lisa Remer, LSW, MA, graduated from Dordt College in Sioux Center, Iowa with a bachelor's degree in social work in May of 1999. She then began working with the Central MN Council on Aging (AAA), as a Senior Advocate from 1999 to 2002. Remer then switched gears and began working for Fairview Home Care and Hospice, where she worked until coming to Fairview Partners. During her time at Fairview Homecare, she also received her master's degree in gerontology from Bethel University. She joined the Fairview Partners team in 2007.

Jackie Sames, RN-BC
Jackie Sames, RN-BC, received her degree in nursing from the College of Saint Catherine and also has her certificate in gerontology. Sames entire nursing career has focused on seniors, from working in the long term care setting to case management with Fairview. She joined the Fairview Partners team in 2004.

Sarah Tellijohn, RN, MSN, PHN
Sarah Tellijohn, RN, MSN, PHN, received a bachelor's degree in journalism from the University of Minnesota, however, she switched gears after a few years and went back to the University of Minnesota to become a nurse and received her master's in public health nursing a few years later. She has experience working in the hospital setting and her public health nursing background is an asset to the Fairview Partners program. She joined the Fairview Partners team in 2008.
