We know living independently is important, so when you join Fairview partners community program, you will receive a case manager who will coordinate your care and who is dedicated to making sure you are receiving the help you need to keep you living independently in your own home.
As a member of the Fairview Partners community care program, you receive all of your primary care from a Fairview clinic physician with the benefit of a community case manager.
Fairview Partners case managers are registered nurses (RN) or licensed social workers (LSW) who communicate with your physician and help coordinate specialty visits for you, arrange transportation when needed, plus, coordinate any services that you need to help you maintain your independence. These services vary depending upon your needs, but, could include: home health care, personal care attendants (PCAs), meals on wheels, durable medical equipment (walkers, canes, lift bars, etc.) and other services.
As a new member in the community program, your case manager will come out to your home for a home assessment. During this assessment, the case manager will ask you questions about your current health needs and find out about any concerns you might have. After this initial assessment, the case manager will implement a care plan with your physician that helps keep you living independently in your home.
Your case manager will check in with you at 6 months via telephone to see how you are doing. These annual in-home assessments and 6 month telephone check-ins will continue for as long as you are enrolled in the program. Additionally, based upon your needs, your case manager will be in contact with you as often as is needed.
Fairview Partners is part of the Fairview continuum of care. The Fairview network provides access to specialists and hospital care. This model keeps communication between providers open to make sure you are receiving the best care possible.
In addition, with the Fairview Partners program, there is virtually no paperwork to fill out and no claim forms to submit.
