We know continuing to live in your own home is important. That’s why at Fairview Partners we are dedicated to helping you live in your own home by making sure you are receiving the help you need to keep living independently.

As a member of Fairview Partners in Your Own Home, you will have access to a care coordinator who works with your Fairview clinic** doctor to make sure you have all that you need to continue living at home.

Fairview Partners care coordinators are registered nurses or licensed social workers who help you get the items and services you need to maintain a healthy life at home. These services and needs will vary for each individual and will be tailored to your specific situation.

As a new member, your care coordinator will come out to your home for a visit and will ask you questions about your current health needs and find out about any concerns you might have. After this visit your care coordinator will talk with your doctor and then coordinate those items and services you need and make sure you are receiving them.

Your care coordinator will always be available to you and will check in with you periodically to make sure you are still doing well and if any of your needs may have changed.  

Fairview Partners is part of the Fairview Health Services network. The Fairview network provides access to specialists and hospital care. This helps to keep communication between your doctor, care coordinator, specialists and the hospital open to ensure you are receiving the best care possible.

In addition, with the Fairview Partners program, there is virtually no paperwork to fill out and no claim forms to submit.

**Fairview Partners does not provide care to the Fairview Clinics in the following cities: Chisholm, Ellsworth, WI, Farmington, Mountain Iron, Nashwauk, Red Wing and Zumbrota. 
Additionally, the following clinics are also not in the Fairview Partners network: Fairview Children's Clinic, Fairview El Centro Clinic, Fairview Lakes Obstetrics and Gynecology Clinic, Fairview Lakes Pediatric Clinic, the University of Minnesota Physicians Primary Care Clinic and Boyton Heath Services or any other Fairview Physicians Association Clinics not already mentioned.
