Fairview Partners offers a program to help keep you living independently while you continue to live in your own home and in your own community

Our care coordinators work with your Fairview clinic doctor** to ensure  you have access to the help and services you need to continue living comfortably in your own home.

Our case managers, along with your doctor, create a program of care that fits your specific needs and is tailored to keep you living as independently as possible.

To be a part of the Fairview Partners program you will need to enroll.

**Fairview Partners does not provide care to the Fairview clinics in the following cities: Chisholm, Ellsworth, WI, Farmington, Hibbing, Mountain Iron, Nashwauk, Red Wing and Zumbrota.
Additionally, the following clinics are also not in the Fairview Partners network: Fairview Children's Clinic, Fairview El Centro Clinic, Fairview Lakes Obstetrics and Gynecology Clinic, Fairview Lakes Pediatric Clinic, the University of Minnesota Physicians Primary Care Clinic and Boyton Health Services or any other Fairview Physicians Association Clinics not already mentioned.
