In order to qualify for the assisted living program,  you must be a resident at one of the Fairview Partners member assisted living facilities, be eligible for Medicare Parts A and B and be at least 65 years of age or older.

Residents with Medicare who privately pay for insurance would need to obtain or already use UCare for Seniors insurance through UCare Minnesota, or a Special Needs Plan (SNP) through Medica or UCare Minnesota.

Residents who are in the process of obtaining or already receive Medicaid (medical assistance) benefits would need to elect the Minnesota Senior Health Options (MSHO) benefit through Medica or UCare Minnesota.
