The assisted living model of care combines the care of the in-house physician and gerontological nurse practitioner (GNP) team from the long term care facility model with the ease of the community model case manager.

If you live in a member assisted living facility and are a patient at a Fairview clinic,  you have the opportunity to join the Fairview Partners program.

Once in the program, you and your family have more options about your health care. You will still be able to go to your primary clinic and see your physician as you always have, however, you will now have access to a care team, that are available to come and see you in your home instead of having to travel to the clinic every time you need to be seen.

In addition, you will have a community case manager who is a registered nurse or licensed social worker, who works alongside your primary care physician and the Fairview Partners care team to help ensure you are receiving the services that you need to keep you living independently in your home.  

Fairview Partners is part of the Fairview continuum of care. The Fairview network provides access to specialists and hospital care. This model keeps communication between providers open to make sure you are receiving the best care possible.

In addition, with the Fairview Partners program, there is virtually no paperwork to fill out and no claims forms to submit.
