If you join Fairview Partners at a member assisted living site you benefit by receiving a Fairview doctor and geriatric nurse practitioner, your care team, who see you right in your own home. Your care team will provide all of your primary health care. You also have the option of seeing your primary care doctor at your Fairview clinic.**

In addition, you will have a care coordinator who will work alongside your care team to coordinate the services you need to help keep you living independently.   

Fairview Partners is part of the Fairview Health Services network. The Fairview network provides access to specialists and hospital care. This helps to keep communication between your doctor, specialists and the hospital open to ensure you are receiving the best care possible.
