If you live in a Fairview Partners member assisted living facility and currently receive your primary care from a Fairview affiliated clinic, you can join the Fairview Partners program.

Fairview Partners assisted living program clinical model of care combines traditional primary care with the ease of a house call by giving you the choice of going out to your Fairview clinic physician or having our Fairivew Partners physician and geriatric nurse pracitioner (GNP) team come to your home. Our physician/GNP team works with your Fairview primary care physician to ensure the best possible care.

As a member of Fairview Partners' assisted living program, you will have a case manager who works with your primary care physician and the Fairview Partners team to ensure that you are receiving the services you need to continue living independently in your home.
