Fairview Partners assisted living program combines traditional primary care with the ease of a house call

With our program at assisted living sites, you have the choice to visit your primary care doctor at your Fairview clinic when you are ill or you can have the Fairview Partners doctor and geriatric nurse practitioner team visit you right in your own home. Plus, you have access to a care coordinator who works to ensure that you are receiving the services you need to continue living independently.

You also have the peace of mind of knowing that our team is part of the Fairview network and is able to communicate with your Fairview clinic doctor so you are always receiving the best care possible.

To be a part of the Fairview Partners program you need to enroll.  To make enrollment as easy as possible, Fairview Partners has Enrollment Specialists who work directly with you, your family and the assisted living site staff to help you join.
