Fairview Partners assisted living program of care combines traditional primary care with the ease of a house call. With our program you have the choice to visit your primary care doctor at your Fairview clinic when you are ill or you can have the Fairview Partners doctor and geriatric nurse practitioner (GNP) team visit you right in your own home.  

You also have the peace of mind of knowing that our team is part of the Fairview network and is able to communicate with your Fairview clinic doctor so you are always receiving the best care possible.

As a member of the Fairview Partners assisted living program, you will also have access to a case manager who works with your doctor and the Fairview Partners team to ensure that you are receiving the services you need to continue living independently in your own home.

To join the program you need to live at a participating  assisted living facility and, you will need to enroll.
