The Fairview Partners program also provides case managers who work alongside Fairview doctors to ensure all your primary health care needs are met while you are living at a participating assisted living facility or even in your own home.

We also offer a Memory Loss Assessment Clinic to help those seniors who are suffering from any stage of memory loss or dementia.

And, for those seniors in need of short term rehabilitation care after a surgery or hospitalization, we provide rehabilitation services through the Transitional Care Network.
